Please click below to download our backpack request form (once you click below for the form, and it opens on your computer, go to "File" and "Save As" and save it to your desktop in order to fill it out).  Next, once the form is filled out please email it to us as an attachment directly at lilysclosetdonates@gmail.com.  In the subject line, please write BACKPACK REQUEST.
